Teamsters Local 120 is pleased to announce that we have secured a $50,000 Voluntary Group Life Insurance Policy for each qualifying member and family. This Group Benefit is serviced through American Pride*, a 100% Union Company and underwritten through The Hartford**. This open enrollment period is time sensitive and expires on November 30th, 2007. We ask each member to complete the enclosed enrollment form prior to November 15th, 2007.

Return you’re completed and signed enrollment form in the postage paid envelope, it must be postmarked no later than November 30th 2007.

• Your $50,000 Group Life Insurance Policy is already approved
• Guaranteed Issue = no medical questions, no exams, no doctor reports
• You cannot be turned down for any reason during this initial enrollment period
• You may choose to accept coverage for spouse and children
• Your Life Coverage stays with you should you leave the Teamsters or retire***
• Monthly premiums are based on the age brackets printed on the enclosed election form
• Additional Beneficiary services are included at No COST
